About the Role:
The Social Care Navigation (SCN) team forms the backbone of Papa, playing a crucial role in enhancing our members’ health outcomes by identifying and mitigating barriers to care. Our team steps in when Papa’s Pals and Care Team agents unearth member social needs or health risk factors during their in-person visits and phone interactions. These insights inform our Social Care Specialists, who reach out to members, providing them with relevant resources. This approach enables SCN to build relationships with members, facilitating the detection of further needs that Papa can support.
In this role, you will be leading the SCN team and function at Papa. This includes overseeing the navigator team, directly managing SCN team managers and indirectly overseeing the navigators who work with members. Additionally, you will be responsible for setting and executing the vision of SCN. To do so, you will work cross-functionally to align on where the SCN team is going and how it fits into Papa’s long term strategy. You will then work within the team and across other groups at Papa to set the clear operating procedures and processes that deliver on company goals. You will make sure the team has the right tools and resources to do their job successfully, and you will evangelize the team internally to ensure goals are being met.
What you’ll be doing:
Strategy development: Partner with leadership to determine how SCN best fits into the broader Papa strategy, leveraging a data-driven approach
Team design: Benchmark, experiment, and iterate to move the team to the most effective operating model
Process development: Develop workflows and SOPs that enable SCN to deliver on program objectives (including collaboration with client care management teams)
Team oversight: Orient the team’s day-to-day work around a core set of metrics that ladder up to the outcomes Papa delivers for clients
Tooling development: Partner with Product teams to build and iterate on tooling that enables the team to efficiently execute their work
Resource development: Develop and maintain a robust set of community based resources via external partnerships and internal team research
Cross-functional collaboration: Represent the SCN team and function internally
Your skills and experience:
10-15 years of experience leading teams in a fast paced environment, with a mix of population health, clinical, operations, and/or other highly analytical and operational domains.
Expertise: Firm understanding of Population Health and/or Public Health principles and practices.
Strategic Thinking: Ability to analyze complex situations, identify key issues, and develop effective solutions.
Team leadership: Have led a team, successfully delivering on team goals while also fostering a collaborative and supportive environment
Data Analysis & Interpretation: Utilize strong analytical skills to interpret data, identify trends, and make informed recommendations.
Communication & Presentation: Present complex information clearly and concisely to diverse stakeholders.
Project Management: Manage projects with multiple stakeholders and deadlines efficiently.
Collaboration & Teamwork: Work effectively with others to achieve shared goals.
Problem-Solving: Identify and solve problems creatively.
